Local Coverage Determinations (LCDs) and Coverage Information
Local Coverage Determinations (LCDs) are developed by and are specific to an individual Medicare contractor to give guidance to providers concerning Medicare's coverage and payment policy for various diagnostic and therapeutic protocols. This area of our Web site allows you to access all active and draft LCDs, as well as retired LCDs. You may print copies for your review and reference. Active LCDs include revision dates, if applicable, to ensure that you have the most recent version. An LCD Reconsideration Process is available for those who wish to request a revision to an active LCD.
National
Coverage Determinations (NCDs) are developed by the Centers
for Medicare & Medicaid Services (CMS) to describe the circumstances
for Medicare coverage nationwide for a specific medical service, procedure
or device.
Home health and hospice coverage guidelines are also discussed in this area of our Web site. Select the Home Health Coverage Guidelines or the Hospice Coverage Guidelines for brief definitions of the home health and hospice benefit, as well as specific coverage criteria and CMS manual references. This information will help you determine what services may be covered by the Medicare home health and hospice benefit.
