Local Coverage Determinations (LCDs) are developed by and are specific to an individual Medicare contractor to give guidance to providers concerning Medicare's coverage and payment policy for various diagnostic and therapeutic protocols. Active LCDs include revision dates, if applicable, to ensure that you have the most recent version.
National Coverage Determinations (NCDs) are developed by the Centers for Medicare & Medicaid Services (CMS) to describe the circumstances for Medicare coverage nationwide for a specific medical service, procedure or device.
