Local Coverage Determinations (LCDs) are developed by and are specific to an individual Medicare contractor to give guidance to providers concerning Medicare's coverage and payment policy for various diagnostic and therapeutic protocols. This area of our Web site allows you to access all active and draft LCDs, as well as retired LCDs. You may print copies for your review and reference. Active LCDs include revision dates, if applicable, to ensure that you have the most recent version. An LCD Reconsideration Process is available for those who wish to request a revision to an active LCD.
