Occurrence Code 42 Omitted

Occasionally, a final claim for a discharge or revocation is submitted and processed without the occurrence code 42 and date.  When this occurs, do an adjustment (type of bill 8X7) to the final claim to add occurrence code 42 and date. Refer to the Claim Adjustments/Cancels Web page.

When it is not possible to do an adjustment, the alternative option is to submit a discharge/revocation notice with the type of bill 8XB (81B or 82B).  However, we suggest that you submit the adjustment to your final claim instead of submitting an 8XB type of bill.  If an 8XB is submitted/processed incorrectly, it may not be possible to correct the hospice record. 

In addition to the usual claim information on a Notice of Election (8XA), it is important to complete the following on an 8XB type of bill.  Note: Do not report the occurrence code 42 with a date on an 8XB type of bill.

TOB field on the Fiscal Intermediary Standard System (FISS) Claim Page 01 Enter 81B or 82B.
STMT DATES FROM field on the FISS Claim Page 01 Enter the start date of the hospice benefit period in which the discharge or revocation is effective.
STMT DATES TO field on the FISS Claim Page 01 Enter the date of the last payable day.  Day of discharge billed at the routine home care rate.
