Medical Records’ Signature Requirements for Providers - Clarification
In compliance with Change
Request 5971, Signature Requirements Clarification, 
the Medical Records'
signature requirements for providers have been clarified; notably that
stamped signatures are not acceptable. The following Local Coverage Determinations
(LCDs) have been updated to reflect this clarification:
- Medicine: Dysphagia/Swallowing Therapy (L923)
- Medicine: Occupational Therapy-Outpatient (L13314)
- Medicine: Physical Therapy-Outpatient (L13267)
- Medicine: Speech Language Pathology-Outpatient (L1078)
These updated LCDs can be viewed on our Active LCD Web page by July 31, 2008.
The effective date for Change Request 5971 was September 3, 2007.
