New CMS-855 Medicare Enrollment Application — Effective July 1, 2008
The Centers for Medicare & Medicaid Services (CMS)
issued a revised CMS-855 enrollment application in March 2008.
Providers and suppliers should begin to use the new Medicare enrollment
application immediately. Effective July
1, 2008, Cahaba GBA will no longer accept applications on the old
2006 version of the CMS-855. For additional information, refer to
the article, Announcing
the Release of the Revised CMS-855 Medicare Enrollment Applications, 
which
was posted to our Web site in April 2008.
