contains an overview of the provider enrollment process, as well as state agency, contractor, and CMS provider enrollment contact information. It also contains the CMS-855A provider enrollment application that can be completed on-line in a PDF fill-able format or downloaded to complete manually. - An informational brochure, Medicare Enrollment for Institutional Providers is also available on the CMS Website.
